ASSESS & GATHER DATA IN ORDER TO IDENTIFY & CLARIFY GOALS
Get started with our comprehensive assessment process. We'll begin with the Initial Assessment & Triage Questionnaire, which offers a broad snapshot of your situation. Depending on your needs, we may include additional questions. Finally, we'll determine your nutritional level. It's all about tailoring a plan that works specifically for you!

UNDERSTAND YOUR GOALS & “BUILD THE STORY”

Let's understand your story and goals together. Through open-ended questions and active listening, we'll identify strengths, limiting factors and any anxieties. Using resources like worksheets and questionnaires, we'll clarify your readiness and establish clear goals with measurable outcomes. Together, we'll outline a plan tailored to your journey.

CREATE AN ACTION PLAN & POSSIBLE “NEXT STEPS”
Now that we've gathered data in Steps 1 and 2, it's time to brainstorm ideas together. We'll identify which skills and practices are most important or urgent to address based on our discussions. Then, we'll map these to create a tentative action plan — developing a plan that moves you closer to your goals.

CHOOSE ONE “NEXT ACTION STEP” & TEST IT

We’ll review the action plan together and choose which skills or actions you want to start with. We'll document your choices and work through it. I'll provide resources to help you stay consistent with your actions, also using the Coaching Feedback Questionnaire to ensure our sessions meet your needs.
OBSERVE & MONITOR WHAT HAPPENS NEXT
Let's gather data on how your actions went. We'll use the Looking Back, Looking Ahead Worksheet, along with any forms you used to track your actions. We'll also consider any relevant outcome metrics, such as Athletic Performance Indicators — this will help us evaluate progress and plan ahead effectively.

USE OUTCOME-BASED DECISION MAKING TO DECIDE WHAT TO DO NEXT
Based on all of the data that we have gathered, we will make educated decisions on what to do next.
